  • Niramaya card or mention Health ID Card number.
  • Self-attested copy of Disability Certificate.
  • All original prescription papers by the doctor.
  • All original bills of Hospital / Medicine / Doctor fee / Therapy fee / Conveyance .
  • Complete bank details
For Enrollment Process:
  • Proof of payment.
  • Below Poverty Level or BPL card
  • Address Proof.
  • Disability certificate
  • Income certificate

  • PwD parents or guardians will have to provide the required paperwork to the closest Registered Organisation (RO) to enrol their child in Niramaya.
  • Once the originals are verified, the Registered Organisation (RO) will complete your Niramaya application online with the help of the scanned documents.
  • Each beneficiary will be provided a Health ID number or card.
